Scheduling an appointment through our website, by phone, or via any electronic form constitutes an agreement to attend that appointment. We ask that you provide at least 24 hours' notice if you need to cancel or reschedule your appointment.
Repeated missed appointments or late cancellations without adequate notice may result in a cancellation fee or refusal of future appointment scheduling at our discretion.
Appointment confirmations, reminders, and follow-up messages may be sent via phone call, email, or text message (SMS) to the contact information you provide.
Program Description: By providing your mobile phone number to Summit Dental Practice, Corp. and opting in, you consent to receive recurring automated text messages (SMS) from us. These messages may include appointment reminders, appointment confirmations, recall notices, post-treatment follow-up messages, and other communications related to your dental care.
Message Frequency: Message frequency varies. You may receive up to 4 messages per month depending on your appointment schedule and care needs.
Charges: Message and data rates may apply. You are responsible for any charges imposed by your mobile carrier.
To Opt Out: You may opt out of receiving text messages at any time by replying STOP to any text message from us. After opting out, you will receive a final confirmation text and no further messages will be sent, unless you opt back in.

No Obligation: Consent to receive SMS messages is not required as a condition of receiving dental services from Summit Dental.
No Third-Party Marketing: Your mobile phone number and SMS consent will not be shared, sold, or transferred to any third party for marketing purposes.
Supported wireless carriers include (but are not limited to): AT&T, Verizon Wireless, T-Mobile, Sprint, Boost Mobile, U.S. Cellular, MetroPCS, and other major U.S. wireless carriers. Carriers are not liable for delayed or undelivered messages.
Where supported by your carrier and device, we may send multimedia messages (MMS). Standard MMS messaging rates from your carrier may apply.
As a dental practice, Summit Dental is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information (PHI) is collected, used, and disclosed in accordance with our Notice of Privacy Practices, which is available in our office and upon request.

Payment is due at the time services are rendered unless other arrangements have been made in advance. We accept most major dental insurance plans; however, it is your responsibility to verify your coverage prior to your appointment. Any balance not covered by your insurance is your financial responsibility.
We reserve the right to require a credit card on file or a deposit for certain procedures. Details will be communicated at the time of scheduling.
